Scoliosis is an abnormal lateral curvature of the spine measuring more than 10 degrees on Cobb angle measurement, often accompanied by vertebral rotation. It most commonly develops during the adolescent growth spurt (adolescent idiopathic scoliosis) but can also occur in young children, adults with degenerative spine changes, or as a result of neuromuscular conditions. While mild scoliosis may require only monitoring, moderate curves (25-45 degrees) benefit significantly from specialized physiotherapy and bracing, and severe curves may require surgical intervention.   • Visible asymmetry of the shoulders, shoulder blades, hips, or waist when viewed from behind

Causes
  • Idiopathic: no known cause in 80% of adolescent cases, thought to involve genetic factors
  • Congenital: vertebral malformation present at birth causing asymmetric growth
  • Neuromuscular: secondary to cerebral palsy, muscular dystrophy, or spinal cord injury causing asymmetric trunk muscle pull
  • Degenerative: age-related disc and joint degeneration causing asymmetric spinal collapse in adults
  • Functional: apparent curvature due to leg length discrepancy or muscle imbalance, not true structural scoliosis
Diagnosis
  • Adam's Forward Bend Test to identify rib or lumbar prominence indicating vertebral rotation
  • Standing full-spine X-ray (EOS or standard) for Cobb angle measurement
  • Risser sign assessment on X-ray to determine skeletal maturity and progression risk
  • Lung function testing (spirometry) for thoracic curves above 70 degrees

Execution Blueprint: Schroth Muscle Cylinder Exercise (Individualized)

  1. 1Stand in front of a mirror and identify your specific curve pattern (right thoracic, etc.).
  2. 2Adopt the Schroth corrected starting position: auto-elongate the spine upward from the pelvis.
  3. 3Breathe deeply into the concave (collapsed) side of the rib cage, expanding it outward.
  4. 4Use muscle tension on the convex (bulging) side to de-rotate and flatten the rib hump.
  5. 5Hold the corrected position for 30 seconds while maintaining rhythmic Schroth breathing. Repeat 5 times.
